Abstract

According to specific medical and social, health related and public policy variations, coordination professionals ensure the return to and/or remain at home of the most vulnerable people. Today, the notion of networks has evolved towards the grouping of professionals into communities, with the objective of sharing and coordinating patient care in order to improve health care pathways through clearer actions and fluider follow-ups.
