In case of clinical suspicion of a prostate malignancy, a prostate biopsy is the most widely used approach to confirm prostate cancer. Unfortunately, exclusion of prostate cancer is not feasible by means of biopsy and also the negative rate remains consistently high. Here, we review the information the surgical pathologist can still gain from a prostate biopsy in absence of overt carcinoma.
